How to use this form set

Record near the event

Bottle amounts, nap times, meals, and toileting are easier to record accurately as they happen. An end-of-day reconstruction tends to lose times and amounts. Use staff initials so the family knows which caregiver can answer a follow-up question.

Keep formal records separate

A daily sheet may mention that a formal accident or medication record was completed, but it should not become the only record of a dose, injury, health change, or custody issue. That separation helps the right record receive the right review and retention.

Protect the handoff

Give the report only to an authorized adult or through the program's approved private channel. Move sensitive family concerns to a conversation. Do not display completed reports where another family can read them.
