How to use this form set

Separate prevention, authorization, and response

An allergy information sheet can identify exposure risks and medication location, but a clinician plan may control emergency steps. A medication form records permission and each dose, but it does not make an untrained employee qualified. A trip form documents consent, but it does not replace a roster or emergency plan.

Write observable facts

Incident and accident reports are more useful when they state the time, place, sequence, observable behavior or injury, immediate response, and notification. Avoid motives, blame, diagnoses, or character labels. Protect other children's identity when sharing a family copy.

Keep the current version available

A detailed plan is not helpful if the phone number, medication location, expiration, trip contact, or permission has changed. Set review dates, remove obsolete working copies, and train staff on where the authoritative record is stored.
