Asking for a full health, custody, insurance, and permission packet at the first inquiry creates unnecessary work and sensitive data. Start with the application, move to registration after an offer, and complete enrollment before attendance. This sequence also makes the meaning of each signature clearer.

Verify instead of assuming

The person paying tuition may differ from a legal guardian, emergency contact, or pickup-authorized adult. Review each role independently. Ask families to provide written updates and give staff one current source for release and emergency decisions.

Pair generic forms with official records

A practical intake packet may still need an agency health form, immunization or exemption record, clinician action plan, custody document, subsidy form, or other official item. Use the checklist to track those records; do not copy an agency form or imply that a generic template replaces it.

Use the application first when a placement has not been offered. Skip it only when your program has already completed that intake step another way.

Can emergency contacts pick up a child?

Only when they are also authorized under the program's pickup record. Do not infer release permission from emergency contact priority.

How should updates be handled?

Give families a written change process, date the replacement, make the current version obvious to staff, and retain superseded records when required.
